Forms
There are two forms – a referral form and a consent form – that need to be completed before the Strengthening Families process can begin.
Consent form
Before engaging with the Strengthening Families process, each whānau or family member needs to sign a consent form. This is to consent to the sharing of your personal information between the government agencies and community services involved.
Complete the consent form, make sure it is signed by all participating whānau or family members, and email it to your local Strengthening Families coordinator.
Referral form
A referral form is required for each whānau or family using Strengthening Families. This records the agencies involved and provides a snapshot of the issues the whānau or family is facing.
Complete the referral form, make sure it is signed by the referring agency, and then email it to your local Strengthening Families coordinator.
