Consent form

Prior to getting involved in Strengthening Families each family/whānau member must sign a consent form. In signing this you formally state you are volunteering to use Strengthening Families.

Please print and complete the consent form (use either the PDF or Word version – the Word version can be completed online; however once completed, you need to save it and then print a copy).

The completed form must be signed by participating family members and then forwarded to your local Strengthening Families coordinator.

Referral form

A referral form is required for each family/ whānau using Strengthening Families. This records agencies involved and provides a snapshot of issues a family is facing.

Please print and complete the referral form (use either the PDF or Word version – the Word version can be completed online; however once completed, you need to save it and then print a copy).

The completed form must be signed by the referring agency and then forwarded to your local Strengthening Families coordinator.

Complaints process

There are a number of ways for anyone using Strengthening Families to formally raise concerns about how things are done. Details of the complaints procedure are available on request from your main contact person, or the local Strengthening Families co-ordinator.
